Overview: The Five Facet Mindfulness Questionnaire (FFMQ) looks at five facets of mindfulness including observation, description, aware actions, non-judgmental inner experience, and non-reactivity. It is a good measure of a where a person currently stands in relation to mindfulness and self-awareness and can even provide insight on the effectiveness of past mindful practices or experiences.

Administration: Self-report

Standardized cadence: Weekly

Supported subscales: Observing, Describing, Acting with Awareness, Nonjudging, Nonreactivity

Age range: Adult

Scoring and interpretation: Answers are scored on a 5-point Likert scale from 1 for "Never or very rarely true" to 5 for "Very often or always true." Some questions are reverse scored including 3, 5, 8, 10, 12-14, 16-18, 22-23, 25, 28, 30, 34-35, and 38-39. Scores are summed and total scores range from 39 to 195. Higher scores are associated with higher mindfulness and self-awareness.

In addition, there are 5 subscales. Subscales and their associated questions include:

Observing: 1, 6, 11, 15, 20, 26, 31, 36

Describing: 2, 7, 12R (reversed), 16R, 22R, 27, 32, 37

Acting with Awareness: 5R, 8R, 13R, 18R, 23R, 28R, 34R, 38R

Nonjudging of inner experience: 3R, 10R, 14R, 17R, 25R, 30R, 35R, 39R

Nonreactivity to inner experience: 4, 9, 19, 21, 24, 29, 33
