Brief Description

The Altman Self-Report Mania Scale (ASRM) is used to assess the presence and/or severity of symptoms of mania - an extremely elevated or excitable mood that is most commonly associated with bipolar disorder but also can be present in a variety of other mental health conditions. The five symptom areas associated with mania assessed by the ASRM are positive mood, self-confidence, sleep patterns, speech patterns and amount, and motor activity. Respondents describe how they have been feeling for the past week (e.g., “I do not feel happier or more cheerful than usual” to “I feel happier or more cheerful than usual all of the time”) on a scale from “do not” to “all of the time”.

Summary of Scoring and Interpretations

The ASRM contains 5 questions, each with five different statements as options. Each statement is scored on a 5-point scale with values from 0 (not present) to 4 (very severe). A total score is calculated by summing the item responses and ranges from 0 to 20. Higher scores are associated with higher levels of or more severe mania. A score of 6 or higher indicates a greater than chance probability of mania or hypomania.

Clinical Considerations

  • Estimated completion time: 1-3 minutes

  • The ASRM is also the same as the DSM-5-Level-2 Mania-Adult measure.

  • The ASRM is significantly correlated with two other gold standard measures for mania, the Clinician-Administered Rating Scale for Mania (CARS-M) and the Young Mania Rating Scale (YMRS).

  • This measure is also available for children ages 11-17.
