Instructions for completing Assessments using an Internet Browser

Assessments and Check-ins are used to gather information related to your treatment from you to your clinician. This information helps your clinician measure your results over time so they can form make informed treatment decisions. πŸ€—

You can complete Assessments and Check-Ins using either Blueprint Client Mobile App or by logging into your Blueprint account in an internet browser at https://client.blueprint-health.com/

Important Notes

  • Sometimes your clinician may send you a one-off Assessment notification outside of your regular Assessment notification schedule. If you receive one of those notifications, you'll notice that when you click the link in the notification, an internet browser will open and you're immediately presented with your Assessments -- no login required!


How to Complete Assessments Using an Internet Browser

  1. Navigate to https://client.blueprint-health.com/ and login using your Blueprint credentials. (If you haven't created your Blueprint account go ahead and sign up first!)

  2. If you have Assessments due, they will appear in the "To do" section. Here you can complete all of your available Assessments by clicking the "Take All Assessments" button. Alternatively, you could complete each Assessment individually by clicking the "Due >" buttons.

    πŸ’‘ If you do not have any Assessments due at this time, there will be no Assessments available for you to take in the "To do" section. Check the "Upcoming" section to see when your next Assessment will become available!

  3. Answer all questions and click the "Submit" button. You should see a confirmation page which means your responses have been successfully sent to your clinician. πŸŽ‰

    πŸ’‘If you accidentally leave a question blank and click "Submit", Blueprint will bring you to the question needing an answer.

  4. Click "Back to Home Screen" and
